I've seen lots of great options for a gas fireplace with a TV on the wall over it - but what happens when you have a pile of other devices you need to connect? PVR, Foxtel, iPad etc. We are building a new house so no restrictions at present.
If you are building you can just have the cords ran in the wall to a point where you want to house all the 'gear' that goes with the to .. Google for design ideas there are plenty!!
For a setup like what you are after, I would seek a professionals opinion as most of the time they can put the AV equipment elsewhere and run the cabling to the TV from there. All your boxes can be neatly hidden away and controlled by a smart remote control.
Also consider that the TV maybe too high above the fireplace. There are products like the EAD from Future Automation that would bring the TV to a more pleasurable viewing level: Future Automation EAD
