Chair and slipcover

Chair looks like Ethan Allen's Devonshire chair. Is that correct? I have one and need to find or make a slipcover for it in order to use it in the nursery, any ideas?

Hi there. I actually got this chair from Babies R Us about 8 years ago. I covered it using a slipcover from Surefit.com.
