What's the height of your bathroom and which geyser type ( horizontal or vertical ) you are using at present, all depends on these two things. If you've not bought the geyser then you can opt for horizontal one for the convenience of installing a false ceiling and then on one wall install the geyser. OR You can have a FLAP customize beneath the geyser for the maintenance and have the false ceiling too. For any query, you can revert back.

The main ceiling will have to be left bare, but you can run wires and recess lights into a 2' wide panel along the border

I Understand. That's a common concern with my clients. But if you choose to have a 3-inch false ceiling, it will not kill the height. you need a minimum of 3 inches for the electrical conduits to pass. And as for the unhygienic environment, it is only created if you have a tray ceiling for cove light. If you install a plain ceiling after a thorough cleaning, there will be no hygiene problem.
