Sam Potter: Any Glazier will supply you with glass for this, Make sure the glass is for purpose. Glass shelves normally have to be shatter proof and usually thick. An idea would be to measure the width of the shelf and then the longest edge of the shelf and then shortest lenght. Make a template out of card finding the center by measuring or folding the card in half and then finding center for the measurement for both the longest and shortest measurements. This will give you the angle for the outside edges. Remember two things: 1: to ask the glazier to soften the edges of the glass and to use padding (cork, felt or rubber) on the fixings if they are metal.
